I'll second Carl's comments... we also use D&B information to help make client credit decisions.
We do not require any kind of forms be completed in order to check the credit. Once you have access to D&B, you can simply run a search of their database for the client (there's no "hard pull" like you would in doing a credit check with FICO score check for an employee).
If you're leveling up your credit management processes, I'd also suggest considering credit insurance to hedge your receivables.

Subject: Potential Client Credit Check
Hello, for those of you doing credit checks, do you use a form where a potential/new client authorizes for you to run a credit check? A lot of clients end up not paying their bills so we'd like to have an idea of the financial stability of a business before working with them. Any ideas?
